DISCLAIMER OF RESPONSIBILITY: By submitting this registration form, the registrant acknowledges that reasonable care will be taken to ensure the safe keeping of all entries during the exhibition (door attendants and white glove attendants while rooms are unlocked), however, participant also understands that there is an inherent risk with participation, and that insurance on entries will be the responsibility of the participant.  Women of the West Quilt Guild recommends you check your homeowner's insurance policy and with your insurance agent to determine coverage for your quilt.

Registrant also agrees that registrant will not hold the Women of the West Quilt Guild, the current Quilt Show Committee, any volunteer working with or under the direction of members of the Quilt Show Committee, nor Plymouth Creek Center responsible for any loss or damage to any entry exhibited by registrant.  Registrant further grants permission for the entry to be photographed, and for such photographs to be used on the Women of the West website with regards to the "Pieces of Our Lives" quilt show.
